• Students: Submit a student self-reporting form. 
• Employees: Submit a employee self-reporting form.
  • Note: This will bring you to a De Anza webpage, but is the reporting form for all FHDA District employees.

For Emergencies Call 911 If you have an emergency or need an ambulance, please call 911. Do not contact the Health Center.

 

Vaccination Policy COVID-19 vaccinations are no longer required within the Foothill-De Anza Community College District.
