Common App & College Report Instructions
Most private colleges and out-of-state universities accept the Common App. Some universities may require students to complete the Coalition application instead of the Common App. If so, please review YouTube videos on how to complete the application.

How to fill out the Common App and submit documents, such as the College Report
College Report Instructions
The Common App College Report process is handled through the Admissions & Records (A&R) Office. A&R can email your College Report to one or more schools if you are applying to colleges or universities through the Common App.
Processing time: College Reports may take 5–7 business days to process.
The Student Section is completed by the student, and the College Official Section is completed by the Foothill College Admissions & Records (A&R) Office.
When you complete the College Report Request Form, it will create and autofill a PDF College Report for you. You will not be required to upload a College Report.
Information Required on the College Report Request Form
Be sure to include the following information:
- Last Name
- First Name
- Date of Birth (MM/DD/YYYY)
- Foothill Student ID
- Accurate email addresses for the schools where you want Foothill College to email your College Reports. If you want your College Report emailed back to you, you may request that as well.
- CAID (Common App ID): The number at the top right corner of your Common App application account
- Foothill College CEEB Code: 4315
- The number of units you have currently completed, the number of units in progress, and the courses you plan to take through the end of your time at Foothill
- All colleges you are applying to: One form may list multiple colleges.
- For a Common App College Report: Choose Yes. Continue with the request form. No document upload is required.
- If your request is for a non-Common App (university-specific) College Report, choose No. Continue with the same request form and upload your university's version of the College Report instead.
Important LimitsDue to the high volume of requests:
We strongly recommend waiting to submit a College Report Request Form until you have earned your desired GPA on your Foothill transcript while still submitting your request before the university deadline. |
University-Specific College Reports
Some universities do not use the Common App College Report and instead require their own college report form. These may be referred to as a Transfer Report, College Official Report, or Admissions Report.

- Cornell College Dean's Reference Form
- Scoir Transfer Report
To submit a university-specific College Report:
- Obtain the university's form.
- Complete the student section.
- Submit the College Report Request Form so Foothill Admissions & Records (A&R) can process and email the form for you after completing the College Official Section.
There are no limits on the number of university-specific College Report requests you can submit. These requests do not count toward the Common App request limits listed above.
