EOPS Student Progress Reports
To ensure you receive timely academic support, all participants must request a progress report from each of their instructors.
How to Submit
Fill out and submit the EOPS Student Progress Report request form. It will be sent directly to your instructors to complete on your behalf. You do not provide a self-report.
When to Submit
For Fall, reports are due Friday, Oct. 23. Requests are typically submitted in the 5th week of the quarter.
