Fine Arts Facility Rentals Application Form

This rental application form is for renting the Smithwick Theatre, Lohman Theatre, Appreciation Hall, and Fine Arts classrooms, only. For all other locations, see the General Facility Rentals website.

Please review the Rental Policies and contact Astrid Chandra, Campus Facility Rentals Coordinator, at fhfacilityrentals@fhda.edu or 650.949.7057 if you have any questions BEFORE submitting a Fine Arts Application Form.

All facilities and services must be specified in the application, which will be the basis for preparing the contract. Once the application has been submitted, you will receive an email from Adobe Sign asking for signature. A non-refundable deposit of $500 is required to reserve a specific date for the Smithwick and Lohman Theatres and must be received within ten days after the application has been submitted.

The $500 deposit must be remitted in person or by mail to in the form of a check, cashier's check, or money order:
