Student Athlete Checklist for New Students

Student-athlete: It is you responsibility to follow and complete this checklist.
Initial each step upon completion.

  1. Fill out the Recuiting Questionaire.
  2. Download and Read the STUDENT ATHLETE CKECKLIST FOR NEW STUDENTS (pdf format)
  3. Read The Foothill Way (pdf format)
  4. Meet with Laurie Bertani, the ATHLETIC COUNSELOR, (650) 949-7164.
  5. Fill out the Foothill College APPLICATION and turn it in to Admissions and Records or fill it out online and fax the signature sheet to (650) 949-7048.
  6. Take the ASSESSMENT TEST for English and Math. Call (650) 949-7230 for an appointment.
  7. FINANCIAL AID – apply for the Board of Governors Waiver (BOGW) recipients need to apply for services with EOPS (Extended Opportunity Program and Services). Call (650) 949-7207 for an appointment.
  8. HIGH SCHOOL STUDENTS: bring an unofficial transcript to Laurie (Athletic Counselor) or bring a copy of your NCAA Initial Eligibility Clearinghouse form.
  9. TRANSFER STUDENTS: send OFFICIAL TRANSCRIPTS from previous college/ colleges to: Laurie Bertani, Athletic Counselor, 12345 El Monte Rd, Los Altos Hills, CA, 94022
  10. Enroll in COUNSELING 50, a required course for all NEW Foothill students.
  11. ELIGIBILITY INFORMATION: Susan Gutkind and Liz Hunter will hold a team meeting before your season begins. If you miss this meeting it is your responsibility to fill out the paperwork.
  12. PHYSICALS - use the physicians provided for your team. If you miss these appointments use your own personal doctor. Make sure you complete the necessary paperwork with the Athletic Trainer.
  13. Enroll in the appropriate INTERCOLLEGIATE CLASS. You will not be able to practice or compete until you register for your specific sport class.
